The Dance Program evaluates only live performances that are open to the general public. Please do not send slides, DVDs, videotapes, or other visual supporting materials.

For organizations submitting the Events Schedule supplemental form (link is found on the left navigation bar), please list your information in chronological order. Each form represents one of your organization's fiscal year of performance information.
